AirAsia flight returns to Perth due to 'technical issue', passenger says 'blade came off turbine'

A passenger says an AirAsia X plane was shaking like a "washing machine" when it experienced a problem with its engine, and was forced to turn back mid-flight to Perth.

Key points:

  • AirAsia X flight D7237 from Perth was forced to turn back one-and-a-half hours into flight to Malaysia
  • Passengers say the captain announced one of the engines had seized, plane started shaking violently mid-air
  • AirAsia said plane landed safely, turn-back due to "technical reason", guests being transferred to new flights

 

AirAsia X flight D7237 took off just before 7:00am (AWST) on Sunday, bound for Kuala Lumpur, but experienced a "technical issue" one-and-a-half hours into the trip.

The Airbus 330 landed back in Perth about 10:00am, with emergency services on site as a precaution.

After the USS Fitzgerald incident on June 17th, I begin to wonder if there's any connection.  Those on board this plane were a highly trained specialist group of military personnel.

"A military plane crashed into a field in rural Mississippi on Monday, killing 16 people onboard and spreading debris for miles, officials said.

A US marine corps spokesman said on Tuesday a navy corpsman and 15 marines were killed.

Maj Andrew Aranda said the flight of the KC-130T originated from marine corps air station Cherry Point, in North Carolina, and was taking people and equipment to naval air facility El Centro, in California, when it crashed on Monday afternoon in a soybean field near Itta Bena, Mississippi.

Aranda said many of the marines were from a unit based in New York with active duty and reserve members. The marine corps said personal weapons and small-arms ammunition were aboard."

MiG-29 fighter pilot ejects from flaming jet during disastrous take-off (VIDEO)

"Footage has emerged online of a pilot ejecting from a Russian MiG-29 after the plane caught fire prior to a routine flight in Belarus.
The incident happened at Bobruisk Airport, southeast of Minsk, on February 23 when the engine of the MiG-29 burst into flames as it accelerated down the runway. The video of the event has just recently emerged."

Search ends for 3 Marines missing after aircraft 'mishap' off Australia

"(CNN)A sea search off Australia's east coast for three missing US Marines was called off early Sunday, the military said.

The three Marines have been missing since what the Marine Corps calls a "mishap" with an MV-22 aircraft on Saturday.
Twenty-three of the 26 personnel on board the aircraft were rescued, the Marine Corps said.
US military aircraft and boats scoured ocean waters off Australia searching without success for the three missing Marines.

The families of the missing Marines have been notified, the military said."

Three passengers and seven of the airline's crew members were rushed to a local hospital after a shaky flight from Greece to Philadelphia on Saturday afternoon.
The shaky in-bound plane encountered 'severe turbulence' right before flight 759 touched down at Philadelphia International Airport. 
Flight attendants had just finished handing out drinks before the plane lurched, causing drinks to go flying and people to slam into the ceiling, according to reports.
Passengers described the event as terrifying and one claims a flight attendant dislocated his shoulder in the commotion."

World’s largest aircraft crashes in Bedfordshire field

"The world’s largest aircraft has crashed in the English county of Bedfordshire, injuring two people. Airlander 10, which is 92 meters (300ft) long and weighs 20 tonnes, careered into a field on Saturday after breaking free from its moorings.
A woman was taken to hospital following the incident in Cardington, where the Airlander is based, and another member of staff was injured. After the moorings broke, a safety system which deflates the aircraft was deployed, according to owner Hybrid Air Vehicles Ltd.

The Air Accidents Investigation Branch is looking into the incident, which was the second time the aircraft had broken free of its moorings. The first time was in August near the plane’s hangar.

Airlander 10 had completed a series of test flights on Friday, hours before the incident. Hybrid Air Vehicles confirmed the crash, saying: “Today there was an incident with the Airlander aircraft at Cardington airfield. The aircraft was not flying at the time of the incident. Our initial assessment is that the aircraft broke free from its mooring mast for reasons that will be investigated.

“The aircraft has a safety system which operates automatically in circumstances of the aircraft breaking free of its mast, and is designed to rip open the hull and deflate the aircraft. This is a safety feature to ensure our aircraft minimizes any potential damage to its surroundings in these circumstances,” it added."

US Air Force air-to-air refueller is forced to make an emergency landing after declaring an on-board incident and diverting to a Scottish airport

"A US refuelling jet has diverted to Scotland after declaring an emergency over the Atlantic Ocean. 
The four-engine Boeing KC-135R jet reported a problem off the west coast of Scotland and diverted to Glasgow Prestwick airport. 
The jet landed safely shortly after 8am.

According to flight tracking radar, the aircraft declared an emergency at 26,202 feet near the isles of Jura and Mull. 
Glasgow Prestwick airport confirmed the pilot of the US Military jet was facing 'technical difficulties' and requested a diversion as a precaution. 
A spokeman said: 'We can confirm that an aircraft diverted to Glasgow Prestwick Airport today, due to technical issues. The aircraft landed safely with no further issues.'
The KC-135R is used to refuel other military jets, especially fighters sent to monitor Russian nuclear capable bombers flying towards Nato areas of interest.  
The KC-135 can refuel fighter jets and other aircraft mid-air and can carry almost 91-tonnes of fuel in its tanks to top off other aircraft.
The KC-135s are based on the original design of the Boeing 707 passenger aircraft although have been extensively modified since entering service in 1956.
According to the US Air Force, the KC-135 can also be used to transport cargo or evacuate medical patients.
In October, a KC-135R declared an emergency moments after departing RAF Mildenhall because of a problem with its autopilot."

3 Killed in Helicopter Crash Into Newport Beach Home ID’d; No Distress Call Made, NTSB Says

"The Orange County coroner's office has released the names of the three victims who died when a helicopter crashed into a Newport Beach home, and federal investigators have said there were no distress call before the crash.

The victims were: Joseph Anthony Tena, 60, of Newport Beach; Kimberly Lynne Watzman, 45, of Santa Monica; and Brian R. Reichelt, 56, of Hollywood, Florida.

Watzman and Reichelt were both employed with The Standard chain of boutique hotels, which has locations in West Hollywood and downtown Los Angeles, a company spokesperson confirmed.

Watzman had worked with the company for nearly 11 years and was serving as general manager for the West Hollywood hotel.

Watzman's brother told KTLA he lives next door to his sister in Santa Monica. Being the general manager at the Standard in West Hollywood was her dream job, Ryan Watzman said.

"I couldn't have hoped for a better sister," he said. "She was beloved by many people, including all the folks she worked with at the Standard."

Helicopter based in La Crescenta crashes near Mountain High in Wrightwood

"A helicopter crashed when the pilot was trying to land near the Mountain High ski resort in Wrightwood on Thursday morning, according to California Highway Patrol reports and the Federal Aviation Administration.

“A Hughes 369E helicopter, tail number N618SG, rolled over when the pilot was trying to land in Wrightwood just before 10 a.m.,” according to Ian Gregor, spokesman for the FAA.

Three males aboard the helicopter were taken by a Los Angeles County air rescue helicopter to a trauma center, according to Gustavo Medina, a Los Angeles County Fire inspector."

2 dead, 100+ injured as passenger and freight trains collide in South Carolina

"Two people have died and 116 hospitalized after a passenger train collided with a freight train in South Carolina.
The Amtrak train, which was operating between New England and Florida, had 139 passengers and eight crew members on board. The two fatalities are both Amtrak employees.

The incident took place at around 2:35am local time in Cayce, South Carolina. The railroad company said that the lead engine and some of the passenger cars had come off the tracks."

The Antonov An-148 was en route to the city of Orsk in the Ural mountains.

Pieces of wreckage and bodies were found spread over a large area. One of the flight recorders has been recovered, officials say."

In one post (I can't remember where), it said the FCO of Rozatom was on the plane that crashed.

He was going to be interviewed re Uranuim-1 and the payments thereof...

 "A United Airlines flight landed safely Tuesday in Honolulu after an engine cover came off during its flight from California, the airline said.

"Our pilots followed all necessary protocols to safely land the aircraft," United said in a statement, adding that all passengers departed the Boeing 777 normally at the gate.

The airliner landed as emergency responders waited nearby, said Hawaii Department of Transportation spokesman Tim Sakahara.

Flight 1175 was traveling to Honolulu from San Francisco."

Train derails, catches fire in New York state

"A freight train has derailed near the town of Attica, New York. Pictures from the scene show plumes of smoke billowing up from the crash.
Emergency crews have been dispatched to the scene, where multiple cars went off the tracks and at least one caught fire. At least two minor injuries have been reported.

A HAZMAT team was called to the scene, according to local media reports citing first responders. Citing officials from Norfolk Southern, the railroad company that owns the train, other reports said the crash involved a car carrier with no hazardous material on board.

The fiery wreckage is near the Attica Central School campus."

'At least 13 individuals died and 15 others were injured when a helicopter surveying earthquake damage plummeted to the ground and crushed them. The helicopter carried Mexico’s Secretary of the Interior, the Governor of Oaxaca, and other top officials.

Mexico’s Secretary of the Interior Rafael Navarrete Prida and Oaxaca’s Governor Alejandro Murat were flying in a military helicopter over the region known as Pinotepa Nacional to survey the damage left by an earthquake that was measured at 7.2 in the Richter Scale. Initial earthquake damage reports point to only structural damages and no human casualties.

For an as-yet undetermined reason, the helicopter crashed from a height of between 90 and 120 feet as the craft was preparing to land. While the top officials sustained minor injuries, the helicopter crash killed 13 people and injured 15 others. It remains unclear how many of those killed or injured were in the craft and how many were on the ground. According to information released by the Oaxaca Attorney General’s Office, authorities at the crash site collected the bodies of 12 victims while another died at a local hospital. The victims are listed as one boy, two girls, four women, and five men."

"A Docklands Light Railway station has been evacuated following an explosion under a train this morning.

Police officers attended Pontoon Dock station in east London to assist rail staff and passengers following a 'minor' explosion under a DLR carriage, which Transport for London said was caused by an electrical fault.

Terrified commuters initially feared a bomb had gone off following the loud bang and large police presence at the scene following the incident.

Two people are said to have suffered minor injuries as one passenger said people had been 'trampled' evacuating the train.

 The line is now back up and running with minor delays after a period of no service on the DLR between Canning Town and Woolwich Arsenal, a line which includes Pontoon Dock, as well as the station for London City Airport. 

A video posted on social media shows smoke coming out from underneath the train carriage while passengers watch on. 

The explosion is believed to have been caused by an electrical fault.

Officers from British Transport Police were called at 11.04am this morning.

Mark Davis, TfL's interim general manager for the DLR, said: 'At around 11am our staff were alerted to reports of smoke on at DLR train at Pontoon Dock."

"NEW YORK, NY — Two New Yorkers, an Argentine woman and two men from Texas were killed Sunday after the helicopter they were on crashed into the East River, the NYPD said. Police on Monday identified the victims of the crash as Tristian Hill and Daniel Thompson of New York; Brian McDaniel and Trevor Cadigan of Texas; and Carla Vallejos Blanco of Argentina, the Police Department said.

McDaniel, 26, was a firefighter with Dallas County Fire & Rescue, the department confirmed.

"He loved his job, loved his friends and family and loved adventure," his brother, Jason McDaniel, told the New York Daily News.

Cadigan, 26, was a video journalist originally from Dallas who worked in New York City for the website Business Insider, according to his public LinkedIn profile. The company was "devastated" to hear of his death, Business Insider spokesman Mario Ruiz said.

"Trevor was an intern at Business Insider until a few weeks ago. He was a smart, talented, and ambitious young journalist and producer who was well-liked and made a big contribution," Ruiz said in a statement. "Our hearts go out to his family and friends."
